Just one, right? When an individual’s dialogue runs over into a new paragraph. And yet, the next paragraph opens with quotations marks again, even though they haven’t yet been closed. Are there any other instances?
This is the only case that I am aware of (unless you count the instance where an individual's dialogue runs over the page, and there's a page missing from your book)
