Does anyone have advice for creating a psychiatric eval for a character? I want to include the entire evaluation/report in my book and have it look realistic (with the appropriate disclaimers, credentials, etc). Obviously the content will be fictional, but I'm trying to get it to look as real as possible. Possibly by using a fillable form or copying a real evaluation (and obviously taking out all of the info to add my own). Any ideas for how to get started on this?
I'd recommend checking out the SCP template, it's great for what you're looking for. The template you want is under the "templates" tab on that page. I've done something similar to this using audio transcriptions instead, although you might want to take a look at real life medical evaluations to get an idea of what the contents may be.
