I wanted to use D2D for printing and import it to Createspace. It seems that I can't finish publishing my book without selecting an ebook arm (kobo, iTunes) and pricing an ebook. But I'm using that 90 days Amazon exclusive, so I can't sell in Kobo or iTunes yet. How can I progress while bypassing the ebook options on D2D?
