Don't think u need either of the hyphens, especially the first, but not positive... I believe the per is designed to stand on its own...
The meaning is perfectly clear. I don't see any errors, though this sort of punctuation is a weak area for me. I feel some mild concern about whether it might clash if the surrounding text is narrative, as opposed to, say, this being an entry in a table.
